One server admin adds the bot. Everybody else keeps writing exactly as they did before.
Click "Add to Discord" and grant the minimal permissions. It creates a #mention-reminder channel on the way in, which is where each person opens their own private thread.
Pick the channels it should watch in the web panel. Threads inside them are covered too, and nobody's DMs are ever read.
Mention Reminder adds a discreet 🔔 to the message so the channel can see it is being tracked - and nothing more. No public "any update?" from a bot in front of everyone.
At the hours you picked - 09:00 and 17:00, say - the bot reminds you in a thread only you can open. The same thread holds a live list you can check any time.

A reminder that has to be dismissed by hand becomes another inbox. This one closes on the thing you were going to do anyway.
Tools that turn messages into tickets only work while everybody keeps turning messages into tickets. This one asks for nothing, because it reads what your server was writing anyway.
A reply, or anything you post in the message's thread, closes it immediately - and closes it for the person who pinged you too.
A 👍 or a ✅ on the original message counts as an answer. Sometimes that genuinely is the whole reply.
A short "ok" or "done" is recognised outright. On the Team plan, AI also spots a longer on-topic answer posted nearby rather than as a direct reply.
A ping whose message is gone cannot be answered - the jump link would 404 - so its reminders close for everyone who got one, not just for whoever deleted it.
Nothing about how your server writes changes. The only difference is whether anyone still remembers the thread on Friday.

On the server's side the difference is zero: nobody installs anything, signs up, or clicks a button they did not already click.
Not "better communication" in the abstract - the specific places a ping goes quiet.
Being in eight servers means eight mention badges and no single place that says what is still owed. Link them and one thread holds all of it, tagged by server.
A code question aimed at one person, posted into a channel doing hundreds of messages a day, is the easiest thing in the world to lose. The 🔔 is what survives the scroll.
When half the pings are @moderators or @support, "somebody will take it" often means nobody did. AI filtering on Team decides which ones needed a person, and reminds only those.
If work already happens in Discord and every attempt at a tracker has been abandoned, this adds nothing to abandon: there is no board and nothing to keep up to date.
Odd that Slack does not have this built in. With all the noise, and dozens of people wanting something from you, it helps to have a watcher for whether you forgot to answer someone.
We keep separate servers for talking to different clients. It is good to have one place that gathers the unanswered mentions from all of them at once.
Finally a tool that does not need its own panel and manual upkeep to work. Teaching people to use yet another external app is a nightmare.
I like how little work it took to start watching messages - and that the bell emoji shows at a glance which message is being watched, so you see right away that something needs a reply.
Discord shows you this list when you add the bot. It is short on purpose, and this is all of it.

No, and it should never be given it. It needs to read the channels you point it at, manage the private threads your reminders live in, and add reactions. It also asks for Manage Channels once, to create the #mention-reminder channel at setup - if you would rather not grant that, create the channel yourself and pick it in the panel.
It is where each person opens their own private thread - the thread is the inbox, and the channel is just the door to it. It gets one message at setup with a button to open yours; that message is posted without notifying the server, and nothing else is ever posted there.
